LSF Information and Rules

**THE REGISTRATION INFORMATION HAS NOT BEEN UPDATED FOR THE 2009 LSF! Please do not register at this time. **

ALL PARTICIPANTS must submit an online entry form at http://serch.cofc.edu/LSF/registration.htm by March 5, 2008.* NEW
All other forms (see below) must be mailed with a printout of your submission and postmarked by March 5, 2008.

• Students in grades 5-8 compete in the Junior Division and must use the following forms and information:

• Students in grades 9-12 compete in the Senior Division and must use the following forms and information:

  • Form 1: Checklist for Adult Sponsor / Safety Assessment Form (REQUIRED)
  • Form 1A: Student Checklist / Research Plan (3 pages, includes Research Plan Attachment & Form 1A - TEAM) (REQUIRED)
  • Form 1B: Approval Form (REQUIRED)
  • Form 1C: Regulated Institutional/ Industrial Setting Form
  • Form 2: Qualified Scientist Form
  • Form 3: Risk Assessment Form (NEW)
  • Form 4: Human Subjects Form
  • Form 5A&5B: Vertebrate Animal Forms (2 pages)
  • Form 6A: Potentially Hazardous Biological Agents Form
  • Form 6B: Human and Vertebrate Animal Tissue Form
  • Form 7: Continuation Projects Form
  • Informed_Consent: Information on collecting human subject research data

  Students are encouraged to use the Intel ISEF Rules Wizard to be sure you are on the right track. This "wizard" asks a series of questions about your planned project and will provide a list of forms that you need to complete.

  Students may also want to use the Science Fair Primer for guidance on starting a science fair project.
