How to declare your Neuroscience Minor
You have three options if you are interested in declaring your minor in Neuroscience.

1. Print this Adobe Acrobat declaration form and fill it out by hand. Please ensure that the form is signed and then take it to either the Biology or Psychology Departmental office.

2. Alternatively, you may click here and complete the interactive Adobe Acrobat application form on your computer (you will not be able to save this form unless you have Acrobat Professional installed). Once the form is completed, you will need to print the form and return it to either the Biology or Psychology Department. Note: This is the PREFERRED METHOD.

Please return the completed application form to the Psychology Department Main Office at 57 Coming Street or to the Biology Department in Room 214, Hollings Science Center.

3. Fill in the online application form and click submit to apply for the minor (Sorry, but this option is not quite ready).
