In order to expedite a termination, the following information
must be filled out on a SEAF:
1. Mark termination on the status line.
2. Enter the student's name, SSN, employing department name
and contact person.
3. Enter the type of employment – WS or NWS.
4. Indicate the date of termination on the line marked "End
Date".
5. Affix appropriate signatures and send to the Student Employment
Programs for processing.
