Application Packet

The application packet consists of three (3) items. These items include a checklist, an application, and a brochure. Adobe Reader is required to view most application packet materials.

  1. Checklist - The checklist is a broad overview of what you should submit to CDS.

  2. Application - Please fill out both pages of the application.
