Guidelines for Documentation
of a Physical Disability


To document eligibility for SNAP Services based on a physical disability, the student submits two copies of medical documentation that meets the following guidelines:

  1. Be evaluated within the last three years (impact of disability or accommodation needs may vary with some disabilities over time and situations).
  2. Be evaluated by a professional qualified to diagnose the particular disability.

Please note: Evaluations administered by a relative are not accepted.
  1. Includes:
  • Diagnosis and date of diagnosis
  • Dates of treatment and treatment provided, or appropriate reports (e.g., audiogram)
  • The functional impact of the disability in an educational setting
  • The evaluator’s signature and credentials

Medical documentation forms are available online or at the Center for Disability Services. The evaluator, in lieu of a written report, may complete such a form.
