______ G)     Have a minimum Major GPA of 2.00 in PEHD/HEAL courses at 
                       the 200 level and above

______ H)     Have aminimum of 122 earned creidt hours

DURING YOUR SENIOR YEAR, IN ORDER TO GRADUATE AND RECEIVE YOUR DEGREE YOU MUST COMPLETE THE FOLLOWING:

______ A)     Request an on line degree check by departmental advisor at 90 
                       hours

______ B)     Request a degree audit form the Registrar's Office at 90 hours

______ C)     Apply for graduation

______ D)     Pay ALL fees
